Like what made them hurt so much, why were they so painful? was it because of what they were seeing, people being killed or injured? or did the powers that be make them painful for a reason? sorry if it's a stupid question, I'm just not understanding how they can be painful. Like getting premonitions doesn't seem like it would be painful at all. I'm just wondering why exactly they would hurt so much.

I think it was more because both Doyle and Cordelia were human - part human in Doyle's case, but his human side was the dominant. Their bodies just weren't made to handle whatever it takes to get the visions from the PTB to their heads. We saw what happened to the 18th century girl who had them - they blew out the back of her head. That's probably what Cordelia would have faced eventually. Doyle, because of his demon side, might have been able to survive with just the crippling pain.
OR, it was all just a part of Jasmine's plan. She made the visions painful so that Cordelia would jump at the chance to become part demon, thus enabling to carry on with her plan to come to earth.

I suppose it's because it taps into a part of your brain you wouldn't normally use and it overloads the system, so it's like when you overload a plug socket and it catches fire.... it's a similar thing with the visions, except the outcome is a severe migraine-type pain.

I thought it was because the person getting the visions could partly feel the pain of what was happeneding- the torment.
I guess the other thing is that supernatural powers in the Buffyverse usually come with a price. Slayers, vampires, half-demons, telepaths, chicks with lightning powers, etc.

Just my own feeling, but I think of it as something unnatural heading into your brain so your body sort of rejects it. Or it's forcing itself into your head.

Yea I think it has something to do with PTB forcing these visions into their heads. Because the visions are something that were given to them, it's not like Drusilla who seemed to be born with some sort of foresight or like Buffy's powers that are somewhat prophetic. The visions are harsh and real so like topher said it could be a part of them experiencing what the victim experiences, like a temporary link to that persons brain. But I just assumed it was because the visions are shoved into their heads, and it's not something a human body can take without pain.

That's the Birthday episode in s3. It's a what if Cordy wasnt there to get the visions from Doyle. So she sees that Angel has the visions instead. Wesley says that it drove Angel insane. Don't know if receiving them actually hurt.

In order to see a vision you have to be able to suspend your normal thinking and normal response to sensory stimuli while they are replaced by whatever telepathic stimuli are coming through. Since it is completely disorienting, the person has to sit or lie down. It did not seem to be connected to the content of the vision as it would be if the sensations were related to whatever pain or suffering the subject of the visions was experiencing. There is always the possibility that the powers that be were not what they appeared to be but were false prophets connected to Wolfram & Hart, the senior partners or the ultimate evil. We never see any proof that the powers that be are heavenly or righteous.
well they must have hurt pretty bad if they were constantly throwing cordy all over the place and having her collapse to the floor in pain all the time.

I think it's partly due to the empathic element of the visions.
Other than that, I've always thought they were like a horrible combo of a night terror, a migraine and a panic attack. All coming from the head and can be very physically distressing/debilitating.
She's being forced to see and hear things that aren't there and they usually look and sound very intense. So I think of it kind of like having a super intense light shone right into your eyes (your unable to turn away of close your eyes to block it out) while someone lets off massive fireworks around your ears. That'd be pretty painful.
